I reread your older posts and it sounds like your trainer in puppy class said Mocha was being defiant last Spring. I think what you are seeing is an escalation of that behavior. She's in charge and she knows it.

I would try the Nothing in Life is Free program.

If that doesn't work, you need to consult a behaviorist. I looked up South Carolina law and you are legally liable for any damages or injuries. If she continues to bite, the State could declare her a dangerous dog and impound her.
