Here's another idea, which works for any sized dog:

When your dog jumps up, catch hold of his front paws ... and keep him up for a while. All you have to do is hold on to his paws for a few seconds and you'll find that he soon struggles to get all four feet on the floor again. Do this each time he jumps up and you'll find he soon stops (although you may end up with a bad back from bending down to catch his paws!)
