Default older yorkie-intermittent diarrhea

Please let me start by saying this is not one of my dogs I'm asking about. An elderly relative phoned for advice caring for her older Yorkie--approx. 7 years old that has had bouts of diarrhea about every two weeks for the past 3 months. They live in a remote rural place and all they seem to have is one very young vet who so far has found in a stool specimen "an unfamiliar bacteria." She was treated with Flagyl, worming medicine and I/B diet and did fine until all the meds were finished and same pattern has returned: Three days no poop, one day normal poop, one day diarrhea, three days no poop, etc. Right now the dog is on a diet of home cooked chicken and rice. Does anything sound familiar to anyone? I'd like to call my aunt with a list of things the vet should do and look for. Thanks.
