I got a butterfly on my foot about 5 years ago. I have 6 total, each having a meaning. Mine on my foot didn't hurt. Neither did the one on the back of my neck. You would be surprised about ones not hurting that you think would. Here is a pic of my tat on my foot, pay no attention to the foot-UGLY!! LOL
