Default Help with bolting out the door

At first, my puppy was afraid to venture outside any door of my house. Now that she's acclimated herself, she wants to investigate everything outdoors. Two times now, with the door opened and my foot guarding the door, she has still managed to squeeze out. I've told her to back away, to stay, but she doesn't listen. I think I'm going to try sound aversion or keep her on a tether at all times because this is scary. The first time she ran from me, then across the street and wasn't listening at all. Most of the time she responds to "come" but the only thing that stopped her was a sound I make that gets her attention, but this time she didn't want to stop at that either. Anyway, does anyone else want to share how they've handled this problem?
