Aww Vicky, I'm sorry you're going through this! I think I would just start with the basics, as if she was a new puppy. You're right in cleaning up really well everywhere, and then keeping her confined to an area. If she does well without accidents in that small area, slowly increase it. If she has accidents, make it smaller. Also pretty much "crate train" her by using the pen/baby gates-take her out to potty after she eats, drinks, and wakes up. PRAISE PRAISE PRAISE for pottying outside, and then play with her before putting her back in the confined area. Eventually she will understand that pottying outside means praise, treats, and playtime, while pottying inside doesn't get such great reactions.
