He is obviously in terrible pain. You need to either find out what is causing his pain and treat him or have him euthanized. It is inhumane to let him suffer.

What other problems does your vet think he has?

When was the last time he had a full geriatric profile? That includes complete bloodwork, urinalysis,and a thyroid panel. Depending in the results, further testing may be necessary.

Seniors (dogs over seven) need this done annually as many health issues can begin as they age. Troubleshooting little issues before they become serious health problems is the way to make sure they live a long happy life.
