Is it possible that you did not make it clear to her just where she is supposed to potty.

I'd suggest putting down pee pads since she is used to using them. Then go back to basics and watch herr when she is roaming free. you have to correct instantly or it does no good.

It is possible that your carpet was peed on by another dog.

If you use a good pet odor remover it should no longer smell to humans, but dogs will be able to smell it.

When you finally have her trained, have it professionally cleaned.
