It is just a blood test. I would personally get it done siince she has been off for so long. The size of the dog doesn't matter. If you have heartworms in your area, most vets would tell you to medicate. Interceptor is okay. They have a dose for dogs 2-10 pounds. Revolution has a dose for small animals but the heartworm protection from it is questionable and I personally don't like it at all.
