I have read you have to pull them down and out. This little puppy was just probably stuck and thats all there is.
I know it is heart wrenching. We had a puppy that was very tiny when I was a little girl, and my dad made me keep rubbing it and then he held it within his hands and brought the puppy over his head and brought his arms down with a mighty swing and he did this 2-5 times and he kept rubbing to warm up the puppy, it came back. It was very weak and I had to make sure it nursed first . It was just a miracle.
You did what was best within the situation.
I am sorry for you loosing the one puppy.
