It breaks my heart to hear of your loss. As the others have said, there is NO way for you to know that she would succumb to the anesthetics. Please comfort yourself and your family and know that we just never know. You did the best you could for her and the best anyone could. You could be one of those that won't address the dental issues and let them be sick and in pain. You were taking care of you baby....and there is no reason for guilt. For some reason, God calls the really special ones home first. I'm not extremely religious but I know in my heart that this is the case.
