We've had bad luck with home made vest harnesses for the very same reason so now we use Step-In Silk Harnesses made by chattiesmom that I swear by! It buckles and then the leash clips on to the D-ring from each side of the harness. It's important that the harness is pretty tight, too.

Although Daisy's harness isn't from Chattiesmom (I'm sure we'll have one some day
) you just described her harness to a tee! I've used this harness on her sooooooo many times...and never not once has she gotten out of it. I don't know what happened! Like I said, it was still attached to the leash and her sweater was inside the harness...like the invisible dog
