I've had that happen with my friends pups. The mom was fine and the other pups were fine too. It was shocking and very sad but sometimes it does happen. When my friend took her to the vet the next day and told him about the pup, he said that most likely there was just not enough room for it to grow (she was a small girl) and that pup was tiny.
