I don't know if Benadryl will help an itchy incision because it probably isn't an allergy but if the vet told you to, I guess it wouldn't hurt to try. Ellie's dose is 1/2 teaspoon and she is just over 5 pounds. Does she have a onesie or e-collar?

If the stitches are dissolvable, she may not need follow-up although they should look to make sure it is healing either way.

When in doubt, have them look at it. It is better than a problem going untreated. I had Ellie's vets look at hers twice before her staples came out and it was no charge.
We tried the onesie thing, woke up the next morning and she was out of it. The incision isn't itchy, just her legs and stuff, I have to give her a bath every week using this soothing shampoo or she itches like crazy, it's the only thing I've found to help. I didn't get to give her a bath before her spay so she's been biting on her legs like nuts. I'm going to call them in a few.
