You might want to research Revolution for the worms/fleas/ticks/mange. It's what I've used for years now and none of my babies have ever had a reaction to it.
I agree with finding a new vet. A GOOD vet won't pressure you into giving vaccines that are known to cause really bad reactions in toy breeds
