This is really a tough one for some dogs - and it takes a lot of time to desensitize them from their fear of nail trimming; you just have to keep doing it and doing it, hopefully with less restraint each time.

I have heard of another member with the same problem who is now using a dremel (small power tool) to file down her baby's nails instead - and she says that once she got him used to the bit of noise, that the dremel is actually easier for him. I don't know that this is for everyone, but it's just an idea.
