I just checked from the site of usda and it stats that

"Most countries including those of the European Union now have a veterinary certificate that is specific to their country.

If there is not healthy certificate for the country you are visiting then you should use the International Health Certificate USDA-APHIS 7001 form. It is officially known as "United States Interstate and International Certificate of Health Examination for Small Animals.

Some countries require that it be certified by the USDA - See USDA certification

My questions are did you have a european passport for your dog when visited Tampa last year? Did you have a health certificate from your veterinary stated that your dog was vaccinated against rabies and your dog is healthy?

I gathered some information from an office here in Belgium and the guy didn't mention about the HC form. I'm confused. Sorry for these so many questions.

Do I still need to have the USDA-APHIS 7001 form even if I have a european passport for my dog, a healthy certificate and a rabies vaccination certificate? Thank you in advance for reading this.
