It's something they use to charge extra money for what they promise will be a very small dog. It's a ripoff, and you could end up with thousands of dollars in veterinary bills. Don't fall for this. And if you don't believe us, check out the "Bad Breeder" page and read all the sad stories of those who have bought these tiny - and very sick - dogs.
