The regular harness goes through the head and locks at the shoulder/chest. Whereas the vest goes through and locks at the chest. I'm my opinion it is harder for them to wriggle their way out of a vest harness. The vest harness also runs small so you have to measure exactly because there isn't much room for adjustments. Whereas the regular harness can be adjusted at the shoulder/chest.
