I doubt if you're doing anything really wrong.
Eleven weeks is still very young & it sounds like she's doing really well.

Just make sure that, whenever she goes outside, you go with her so that you can see if she actually does anything or not. The minute she does, even while she's still squatting, start telling her what a good, clever dog she is. Make a huge exaggerated fuss about it, that way she really associates the praise with the actual doing.

If you find puddles in the house, don't scold her. That will just upset & confuse her, just clean it up & remind yourself to watch her a bit more carefully in future for signs of her needing to go out.
