I didn't get the impression the guy couldn't afford to take care of this dog...rather he chose not to.
We have a pet supply place here...that offers a wellness clinic, once a month. They give free vaccinations, free heartworm checks, flea preventative and heartworm preventative. Or if your animal is ill...they will exam them. I've seen plenty of people there...who may not be able to afford treatment for their animals...but chose this option.
