If your boy is actually having pee accidents you should use a pad with them, if he is only marking with it on, you don't necessarily need a pad if the belly band is absorbant. I always thought that you had to use a pad with them too, but then I found out that my mom doesn't use a pad with her male anymore because her boy has gotton to the point where he doesn't full out pee in them, just marks a drop here and a drop there. I put an extra layer of absorbant fabric in the middle of the belly band and it does the job for her male without the pad. If it is for use for a dog who actually pees in them, then a pad would definately be needed. But for light marking you can get away without using a pad.
