If your dog is healty and has no history of being sick at all. I wouldn't do the bloodwork this is just my opinion. If you had a dotg 7+ years it would be different. I would call other vets and see what their policy is. But to me if it is that big of a difference in price and the vet had no concerns I wouldn't do bloodwork. Of course, they are going to tell you there is always a risk with any type of surgery. The $210 does that include the laser?
In GA there is an organization called spay GA that you can purchase discounted certificates and they give you a list of vets that accept them. I had our Chocolate Lab spayed a couple of years ago and I paid $60 for the certificate and had to pay and additonal $50 I had a umbilicial hernia repaired at the same time and of course they went ahead and gave her antibotics just as a precaution. This was still about $100- $150 cheaper than just going to the vet without it. I live in the Atlanta area and things tend to be a little higher.
