Unlove She was doing so well, what happened?

My girl is 7 months old now and is NOT completely housebroken and its driving me BONKERS!

She was doing so well after 4 months I was quite hopeful. By 5 months she was eliminating mostly outside maybe 1 accident every 3 days. By 6 months it was once a week or so. but now shes having an accident almost everyday (she even POOPED inside yesterday). It's been 3 days in a row now. What happened? I was almost there I thought.

It's odd to me bc at SO's place she hasnt had an accident for 3 months or so. She'll scratch at the door to go out. but at my house sometimes shell bark to go out, sometimes shell just walk out and you have to follow her to the door. What gives? His place is significantly smaller than mine is that it? He's in a 2/2 while we're in a 5/3. Over the last week, Ive blocked off half the house. Before I just had all the doors closed. At our home if she has an accident its always in the same two places...dunno if that vital info but thought i'd throw it in there. It's quite frustrating as I know she should be trained by now. I feel like ive failed her. SO says its bc I dont discipline her (not hit her but tell her nuh uh, etc). This is usually bc i dont catch her in the act. I read here that if I dont actually catch her she wont know what im yelling about I'll figure out that shes had an accident about 2-3 mins after its occured bc shell walk up to me and jump in my lap with this sheepish look on her face. I feel like she knows what she did was wrong...

Any light on this subject would truly be appreciated. Anyone experience something like that? Ive had her since Feb. so its quite embarrassing that shes not housebroken yet.
