I would speak to the vet. We use petzlife spray every week and Chewys teeth seem ok. shes 2.
Start out brushing daily with a doggie toothpaste and use the petzlife every third day or so...you should see a change pretty soon.
If they dont let you brush in the back (like mine) just spray a little more back there or put the gel...
