Midwest Airlines is an exception to this rule - They asked for her health certificate when we went to KC. Not only do they require it but they are also picky about it. My vet forgot to date the certificate (2 days old) and they made me call them and fax them a dated certificate before they let me fly.
Taken from Midwest Airline's website - Vet Health Certificate. Upon check-in, you must present a veterinarian health certificate that includes current vaccination records and is dated within 10 days of travel.
I don't have the opportunity to fly Midwest often, I wish I did as they are very comfortable. One interesting note on them - They have a frequent flyer plan for pets. Six Legs and you get a free round trip ticket for them
