I use to be a vet tech years ago and neutering is so quick and easy. We do put them under, but the operation just takes minutes. To neuter a cat is even faster and you will be surprised how fast.
I think dogs do have a higher pain tolerence than humans. Your vet can give something for pain though just in case, just ask. Something mild though.
