I am so sorry your having to experienceall of this stress. I am thinking that maybe you could go the route of having him as a service animal. What you could do is go to your primary care doctor and tell them you need a letter for you dog stating the he is a service animal. You do not need to tell the doctor about him needing surgery. Tell the doctor that The dog helps you with your emotional needs, and anxiety. The dog provides you with keeping your emotions and feeling balanced, and you less stressed out because you have him.
i hope that this does not seem dishonest or anything, but, If it could help you and Your Dog, perhaps it is worth a try.

My Yorkie pup was got for a service dog, and it actually was not too hard to do. Service dogs, or emotional support animal are there because they serve a a unquie purpose. If your doctor is willing to write the letter then I hope he could have the surgery.....
