Also make sure you use some kind of odor remover on his accidents. You said that he always pees in the same spot and that is probably do to the fact the scent is there and they like to go where they've gone before. I would imagine pooping by the dinner table was an attempt at attention. To dogs and most people if they crave attention negative or positive will work just the same. He was happy about distroying his pee pad because he probably looked at that has killing something just for you. I used to have a dog that would bring me dead mice and he'd be so proud of himself. Also when they're bored they tend to make there own entertainment and I'm sure destroying that pad was a blast.
