I would put my dog on the same food as Ruthie and maybe one of the supplements like Denosyl or Denamarin, and do a BAT in 30 days. If it's normal, great news!! If it's still elevated, you're probably dealing with MVD.
Make sure that before the next BAT, you do not give any meds or flea/heartworm stuff within two weeks of it, so that those things can't throw off the numbers.
This way, you will have a much better idea of what's going on...

I was wondering about that… Oscar just had his vaccines yesterday and is supposed to go in tomorrow morning for the BAT — I guess I should cancel it and wait a bit? I am also supposed to be applying his Advantage tonight, but maybe I shouldn't?
