Just a quick thought. We had a litter and the LAST puppy was very large. She was breach and half-way out of the birth canal and wouldn't go any farther. I usually don't break the sack until the puppy is out because I'm afraid they will start breathing with the head still inside mom. Finally I got a little panicky and broke the sack. The release of the fluid around the puppy was just enough to allow it to slide out on the next contraction. I didn't know about K-Y at the time to unstick a puppy, but this worked.
