chewy used to do that too...and it turned out not to be a medical prob at all, although it may be for yours...
what we did was switched him from a collar (even though we had a cute one) to a little harness...this was when he gets active and is moving around there is not pressure on his neck and then we walk him and tug it doesnt hurt him, it actually makes tugging a lot more successful!
