That's a tough one. All I can think of are a few questions or suggestions.
When he started that, did you take him to the vet or some other place that he wouldn't like that was overcrowded or in very heavy traffic with lots of semi-trucks or honking cars?
I would try again taking him on some very short, pleasant trips to quiet places like parks that he would connect to enjoyment.
