Try putting the leash & harness on him (in the house) and sit on the floor with him. Hold out a treat to make him come to you so that he will associate it as a positive. If he does have a good reaction to doing this after a few good try's you can walk around the house with him and give him praise and a treat until you feel he is ready to head outside for a walk. I bet he will love the outside walks once he gets used to the leash and harness. My way of thinking walking is one of the best trainers for a pet. It gives them a new adventure, different routine, exercise more, etc. in turn he may stop doing this in the house..

I know this will sound sort of crazy but can you blocked off the table in the kitchen so that he can't get under it. When we were training our little one we had cardboard blocking our table and other areas of the house until he was trained. Let us know how you both are doing.
