As for the bells, my Petsmart Trainer said that it is a very helpful tool when it comes to potty training! He said to just ring it everytime you're taking her outside to potty (not to play/walk/etc). Eventually, I think your puppy will get the hang of it. I think it's even better if you start it when she's that young! I have to still potty train mine... but just sharing a tip from last week!
