I know it is scary, but please try not to go there yet (lol..I know..not a chance).
It is very likely not a shunt or MVD.
Dogs with MVD are often asymptomatic and it is caught during an optional BAT.
Not all dogs with shunt or MVD will have neurologic symptoms.
The skinniness and being a bit quiet would concern me. The turning the head away to avoid a kiss isn't head pressing.
