Lulababy, sounds like you know more than you're giving yourself credit for.
Sounds like you need to step back and sort of start over on a few things. In potty training, I always say that a few months of dedicated hard work will most likely result in years of stress-free living! If you're having trouble, you have to tackle it with 100% attention to the pup unless they are crated or penned. You can gradually expand their free reign area as they become trustworthy. As an example, Coby just turned one year, and he is 100% trained but only in the rooms he has free reign. We are still in the process of adding rooms - but only by watching like a hawk during the time he's in there and walking him back to the puppy pad to remind him of the "path". Hopefully, one day soon, he'll have free reign like our older dog (who by the way is aggravated by all the new puppy gates all over his freakin' castle!
