years ago I had a sheltie that contracted mange. He smelled bad and lost hair and itched really badly... we couldn't see it even with skin scrapes at the vet's. i took him to his breeder and she dipped him for mange twice and he got better almost immediately.

i don't know how they'd do this for a tiny yorkie, but it really sounds like she might have that.
