At three months old your puppy can only go 4 hrs at the most with out using the bathroom, and that is pushing it. She may be afraid to jump from the bed at night to use her pad. She is better off secluded in her own sleeping area or you try to be aware when she starts moving around take her to her pad. I really don't expect much until they are almost a year old. Mine still will have goofs, when they have to go they have to go. Sometimes we get busy and not realize how much time has passed.
