The t-dresses fit the following:
Length Chest Neck
XS 7-7.5 11-12.5 8.5-9.5
S 8-9 12.5-14 9.5-10.5
M 9.5-10.5 14-15.5 10.5-11.5

So I would say a small. Zoe is also 5lbs and the small fits her perfectly.
