Most vets put them under because it is really the only way to do a good job and get into all the places they need to.

It is recommended that they are cleaned 1 or 2 times yearly.
If the teeth are kept very clean it doesn't need to be done nearly this often.

Allowing tartar to build on the teeth can weaken the heart and kidneys over time then there would be organ problems to deal with. Cleaning the teeth is definately the way to go if there is build-up.
