I'm sure someone else can explain this better, but here goes...all dogs have glands just inside their bum that are supposed to empty themselves during a bowel movement. Very small dogs often have bowel movements that are too small to stimulate the glands, so the glands get engorged with smelly stink, and have to be expressed manually.
Most groomers will do this for you if you request it. Vets have to do it if you request it. My oldest daughter will even do it if she's home visiting (she's pre-med, nothing grosses her out)...but I just can't do it myself...I wish I could...but I just can't go there.
