This is what I used and found the info for you. Call them they are very accomodating and also allowed me to get Koda out for a few minutes.

Only one pet per passenger is allowed in the cabin. Your dog's carrier must fit completely under the seat in front of you and remain there at all times aboard the aircraft. The maximum dimensions of the pet carrier are 22"x14"x9" and it must be carried in lieu of a carry-on bag. Only soft-sided kennels are allowed in the BusinessFirst cabins of Boeing 767 aircraft and the kennel must not exceed 16"x12"x8.75". No oxygen will be administered to a pet in the event of an emergency.

There is an $80 USD service charge each way ($160.00 for round trip travel) for a pet traveling with you in the cabin. Arrangements for your pet must be made in advance. You can book your pet's trip online at Continental.com or by calling Continental reservations.
Pets Checked with Baggage:Continental is committed to providing a safe and comfortable travel experience for all of their customers, including pets. Because they want your pet's flight to be stress-free, they have enhanced their animal acceptance policy to transport pets as cargo. This allows pets that previously traveled as checked baggage to experience increased safeguards, keeping them safe and you reassured. If your pet is traveling in the cargo area on a Continental flight, you will have access to a dedicated 24-hour Live Animal Desk, confirmed booking prior to departure, and the ability to track and trace your pet from origin to destination. Furthermore, weather conditions will be constantly monitored at all times and your pet will get personal handling in climate-controlled vehicles whenever he will be exposed to temperatures above 85�F for more than 45 minutes.

Pet fees range from $109 for animals under 10 pounds to $459 for animals over 150 pounds. To find out the cost for transporting your animal in the cargo area, or make a reservation, call Continental's PetSafe Desk at 1-800-575-3335.
