Yes, you can get a combination vaccine without the lepto. That's what toy breed dogs should get unless there is an outbreak of lepto in your area. If there is and your vet advises you to get lepto, make sure it is given separately from the other shots with several weeks in between. Fort Dodge makes a single dose lepto vaccine.
The scary thing about lepto is that the reaction usually doesn't occur with the first shot so people think their dog can tolerate it. It's usually the second or even third shot that triggers the reaction.
