Of course its possible with unethical breeders
Just let the breeder know your concern and they should be very reassuring (if not find another breeder). They should let you know how they personally can tell the puppies apart (personality and looks) and like others said to get updated pictures of the puppy as they grow.
And of course be sure to post a pic of the baby you choose
