I think that you should get a book on puppy training, they have great information on how to potty train using different methods. I take Jeter outside to potty and we have only had a few accidents, he is 16 weeks now and still getting the hang of things. We learned to watch for signs that he had to go outside, so that has helped us.
If he is in the crate Don't let him out to run around first,take him out and say potty or go potty to him. Also if you are using both a pee pad and out side training, he is probably confused on what to do! Good luck, it takes time!
