Back pain can be very hard to diagnose. Ask your vet if he'd mind taking one x-ray of his back and neck. Or perhaps just palpate his entire back and neck and see if he has pain. I've had dogs that had very minor 'accidents' and reacted pretty severely with pain in their necks and backs over the next few days. One even had a more severe issue that eventually required surgery that a small tumble triggered.

Hopefully it's just fear (behavioral) ...but have him thoroughly checked out.
