Simon had something similar on the top of his ears. Our vet said it was most likely caused by rough play between he and Hallie and was just a little scratch but since the ears are so sensitive they tend to scab over from the smallest of scratches.
I think you should consider switching vets just for peace of mind. It's such a relief to have a vet you trust and respect
