No, it doesn't hurt them to swell so big and yes, it is normal. If you don't need the diaper, I wouldn't put it on her. Let her clean herself. Her twat will not go back to the size it was before she came in heat. Also, if you noticed her teats, they are bigger than they were before she came in heat and they also will not go back to their original size. After they have puppies their twats do not go back to original size either. The swelling is indeed so that the male may enter.
