Your timing has to be off. We had issues with Sam pooping in the house right after she'd gone outside and it was always the same time of day (morning and afternoon). So for a month or so I sat outside with her as long as 20 minutes until she went and THEN she was free to run around inside. Now she asks (FINALLY) but it was touch and go there for a while. If I'd had enough of sitting outside I would give her a time out by hooking her leash on the doorknob so she could pretty much lay on the rug and hang out but not run around. Then I'd keep taking her out until she went. Eventually she got the idea that she doesn't get to have fun UNTIL she poops outside.

Also, Loki had a plastic crate he HATED when he was a puppy. He'd pee in there the second we put him in it and then we'd put him in the wire one while we cleaned it up. He was fine with the wire one. If she is afraid of the crate, just drop lots of treats in there and reward her for being in there. I find mine sleeping in their crates all the time.
